This document provides a description of the work undertaken by the DHI, led by the Glasgow School of Art (GSA) in partnership with NHS Lanarkshire, to innovate care for people living with type 2 diabetes. The project was undertaken between December 2017 and May 2019, with the purpose of informing the strategy for delivering person-centred care for diabetes type 2 in Lanarkshire. The report is accompanied by a short film illustrating the co-design process, and a policy review undertaken by DHI colleagues based at the University of Strathclyde.
